Champlain Community Health Centre Executive Directors' Network

The Champlain Community Health Centre Network consists of 11 Community Health Centres (CHCs) and is designed to increase collaboration and partnerships related to the delivery of primary health care services in the Champlain region. The Champlain CHC Executive Directors meet regularly to focus on strategic priorities and to build skills and connections.

Champlain Local Health Integration Network

Local Health Integration Networks (LHINs) are non-profit organizations designed to plan, coordinate and fund the delivery of health care services across the province of Ontario. The Champlain LHIN, one of 14 LHINs in the province, encompasses the City of Ottawa, Renfrew County, the Eastern Counties of Stormont, Dundas & Glengarry and Prescott & Russell, as well as the northern parts of Leeds, Grenville & Lanark.

Champlain Regional Stroke Network

The Champlain Regional Stroke Network (CRSN) operates as part of the Ontario Stroke System and is accountable for providing leadership in the development, implementation, and coordination of stroke care throughout the region and across all points in the care continuum (health promotion, primary and secondary prevention, pre-hospital, acute care, rehabilitation and community reintegration including long-term care). The CRSN is housed at The Ottawa Hospital Civic Campus.

Eastern Ontario Health Unit

The mission of the Eastern Ontario Health Unit (EOHU) is to promote and protect the health and wellness of its community by preventing diseases and injuries, and by advocating for a safe and healthy environment through strategic partnerships. The EOHU serves residents of the counties of Stormont, Dundas, Glengarry, Prescott, Russell and the City of Cornwall.

Healthy Active Living & Obesity Research Group, CHEO

The Healthy Active Living & Obesity (HALO) Research Group is housed within the Children’s Hospital of Eastern Ontario (CHEO) Research Institute. HALO’s vision is to provide international leadership and research excellence as it relates to healthy active living and obesity in children and youth.

Heart and Stroke Foundation

The Heart and Stroke Foundation is one of Canada’s largest and most effective health charities. The Foundation aims to build healthier hearts and minds across Canada by bringing life-saving knowledge to the communities it serves, through local area offices and the healthcare providers they support with education and resources.

Leeds, Grenville & Lanark District Health Unit

With its main office in Brockville, the Leeds, Grenville & Lanark District Health Unit offers a wide range of services to promote healthy living and healthy growth and development, prevent illness and injury and control communicable diseases in the community. Services are available to individuals and groups of all ages in a variety of places.

Ottawa Public Health

Ottawa Public Health (OPH) provides public health programs and services to individuals and communities while advocating for public policies that make the city and its residents healthier. OPH is a teaching health unit and works with all post secondary educational institutions in the area. OPH is governed by the Board of Health, which is comprised of elected members of City Council and public members.

Renfrew County & District Health Unit

Renfrew County and District Health Unit serves the County of Renfrew, the City of Pembroke, the District of South Algonquin and most of Algonquin Provincial Park. Its programs and services address the prevention of the spread of diseases and the promotion and protection of the health of residents.
